Signature Verification Statement Form

If we have determined that the signature you provided on your vote-by-mail or conditional/provisional ballot identification envelope does not match the signature(s) on file in your voter record, please complete the Signature Verification Statement form and return it no later than 5:00 p.m. two days before certification of the election. Please note that certification must occur no later than 29-days after the election but is not a set date. Please contact Yolo County Elections to find out more so that your vote-by-mail or conditional/provisional ballot can be counted.
